    Background: On the night of 15 April, Beech 200 VH-IBC (BB-74) was operating a night freighter flight from Mackay to Brisbane. At top of descent, Captain Graham Syphers found that the elevators and trim tab would not move and it was deduced that they had iced up. By reducing power, the aircraft descended into warmer air where the ice melted and elevator control was restored. Despite an exhaustive investigation, engineers could not positively identify the source of the problem. On 20 April while inbound from Lord Howe Island, the problem returned. On this occasion, the elevators had to be freed with manual force. On arrival at Brisbane, engineers found a large block of ice over the elevator bellcranks in the rear fuselage. During subsequent rectification work, engineers also found small cracks either side of the rear pressure bulkhead. These cracks were repaired with a series of external doubler plates around the rear fuselage. This work resulted in VH-IBC being out of service for nearly a week, hence the charter of VH-MXK.   Ferried from Adelaide to Cairns by the late John Newmarch to operate a contract in support of the Ok Tedi gold and copper mine in Papua New Guinea. The aircraft operated five times a week Cairns-Daru-Tabubil-Cairns, a round trip of approximately 7.5 hours. John Newmarch was the base manager and check pilot for the operation. Robert Maddern was one of five pilots who flew the aircraft on this contract. He first flew VH-MXK in November 1986 and his last flight was in April 1989. The aircraft continued to operate in North Queensland after the Skywest takeover until approximately 1991. (Source: 7)

  The aircraft was based in Broome in the early to mid 1990s where it was owned by Skywest Aviation which then became Pearl Aviation following the loss of the coastwatch contract to National Jet. MXK operated a mining contract for Western Metals who owned the CADJEBUT lead/zinc mine about 50km to the east of Fitzroy Crossing. The schedule involved moving workers and freight between Broome, Derby (not Curtin) and the minesite on a Monday to Friday schedule. The contract averaged 800 hours per annum. Around 1995 the aircraft was modified to add wing lockers and 14 passenger seats. VH-MXK was later replaced by a Metroliner as extra capacity was required. (Source: 9)

  Departed Brisbane at 0022Z for Cobar, NSW where it landed at 0249Z. After refuelling, it departed Cobar at 0400Z only to return at 0450Z after an in-flight failure of the right propeller gearbox. Pearl Aviation engineers travelled to Cobar to perform an engine change with a replacement engine which was trucked from Sydney. The propeller was trucked to Brisbane for overhaul because it had suffered sudden stoppage and metal contamination. As no spare propeller was available, the aircraft was grounded at Cobar pending return of the overhauled propeller. (Source: 5)
